How it works

Getting started with a care package:

Initial Referral

You may have decided that you need some help at home or have been referred to us by Social Services or NHS: either way any information we know about you will be dealt with sensitively and in confidence.

Before we arrange any care we will need to send a senior member of the care team to talk to you. You can invite anyone you wish to the meeting: We need to make certain that the services we are providing are the right ones for you.

Care Plan

Once we have met you and discussed your needs, with your help, we will design a package of care.

Within the care plan will be the preferred visit times and the tasks you’ve agreed upon to be carried out within those times. We also try our hardest to make sure that you have consistency with the carers that visit because we want you to feel happy and safe whilst in the care of our team.

Reassessment of Needs

Over a period of time people change and so do their needs. Some will need more help and others less. All care neesd will be reviewed annually but if care needs change between times we will come out ato discuss nd reassess your needs, regardless of the amount of time since the last planned review.

At any time, if there is anything about the care you would like to discuss or comment on you are welcome to contact the head office and speak to a senior member of the care team or manager.
